The Institute of Medicine (IOM) in its 1999 seminal study, To Err is Human: Building a Safer Health System, estimated as many as 98,000 people die every year from medical errors in the U.S. at a cost of $29 billion. A more recent study published in the Journal of Patient Safety suggests that that number may be closer to 400,000.
